There are several types of chinese visas. This space will indicate which one you have been approved for. The most common visas are L (tourist visa) and F (business visa).
Date of Entrance Validity:
Depending on the visa you have been issued, there is a specific window of time available within which you must enter China. The expiration of this time frame is in this space.
Date of Issue:
This space will indicate when your Chinese visa was issued by the authorities.
Traveler's Name:
Your full name is placed here. If you have both a chinese and an english name this space will hold the chinese name.
Traveler's Birthdate:
Your birthdate will most likely be in DD/MM/YYYY format.
China Visa Remarks:
This space is available for any remarks the issuing embassy needs to make on your visa.
Number of Allowed Entries:
There are three possibilites for the number of entries. Single, Double, and Mulitple. The number of entries describes how many times you are allowed to enter the country. Traveling to Hong Kong is considered an exit, so to go back into mainland China would require double entry.
Duration of Validity:
The duration of Validity states how long you are permitted to remain in China after you have entered the country.

States which consulate gave you your passport
